The worst part is the coffin outline. The lines are too straight. Everything else is decent, but those straight lines contrast with the unevenness of everything else.

the only problem i have with it is the shading on the coffin, would have been better just completely black and white

Like a few others, I too thought it was a cunt surrounded by the bush. But, I guess that was Metallicas or whoever designed that covers intention. To make it first appear like something else. A closer look at the cover and got to know it was supposed to be a coffin. Makes sense since the cover on Load was supposed to be semen. Dunno if the clenched fist in St.Anger was supposed to symbolize someone masturbating. I agree that it is good to see the old logo back. Metallica have never had the best of covers for their albums anyway, so I am not surprised I don't like the cover. Stupid cover and equally stupid name for an album.
