Apart from some the real weird ones, I feel Pantera's metal magic definitely classifies as one of the worst ones.

The album cover for Black Sabbath's Born Again is pretty brutal

From Wikipedia:

The album cover, which featured the image of an infant with horns and vampire fangs, was designed by Steve Joule. He also handwrote the lyrics that appear on the inner bag. The deliberately lurid design was submitted by Joule in a vain attempt to be rejected from this design commission, as he was also retained on a lucrative contract by Ozzy Osbourne's organization for his sleeve designs. To Joule's horror and surprise, Tony Iommi and Geezer Butler approved the image for the album. Gillan and Ward were not present when the decision was made, though they later noted that they hated the finished cover. Joule reports that he was drunk and high on speed when he drew the finalized designs for the album. The cover is hated by many fans, but also has a cult following (much as the album itself), most notably with Max Cavalera and Glen Benton both stating that it is their favorite album cover.

Eh, I've seen far worse than that Born Again cover. That album fucking rocks something fierce though.

That Fates Warning cover on the other hand, yeesh. That's just plain ugly.

That's supposed to be bad though isn't it. I listened to some Acid Bath about a year ago and thought it was unlistenable, so maybe I need to try it again.

I'd have thought most people would like a mix of Kyuss style doom and frantic death metal. Mintrude, you'll love it.

I'm checking out some Acid Bath now, it's certainly a lot better than I remember. Interesting fact: the cover for WTKSP was done by John Wayne Gacy

I always really liked Gacy's drawing. It seems really demented to me. Notice the A Clockwork Orange clips in the song 'Cassie Eats Cockaroaches'.

Penetrator's cover is kind of gay. Homosexual gay not lame gay.
